Medicare Part D

 

This information is provided to you in parts, depending on your financial circumstances. There is a table with numbered columns that summarizes the rules about the Medicare Part D coverage. In addition:

  • If you are an individual making $1197 or more a month, your information is in Column 1. For a more detailed explanation, click here.
  • If you are currently enrolled in AHCCCS, QMB, SLMB or QI-1, your information is in Columns 2, 3, or 4. For a more detailed explanation, click here.
  • If you are an individual making $1024 or less a month, and not on AHCCCS or QMB or SLMB or QI-1, your information is in Column 5. For a more detailed explanation, click here.
  • If you are an individual making $1196 or less a month, and not on AHCCCS or QMB or SLMB or QI-1, your information is in Column 6. For a more detailed explanation, click here.

Medicare Part D Hotline in Spanish - National Alliance for Hispanic Health has created a bilingual hotline to assist individuals with the Medicare Part D enrollment process. Individuals can receive 1 on 1 counseling, assistance applying for low income subsidies as well as finding a plan to meet their individual needs. Hotline is available Monday - Friday from 8:00 - 6:00 p.m. ET, 6:00 a.m. to 4:00 p.m. Arizona Time. The number to call is 1-866-783-2645.

ATTENTION AIDS Drug Assistance Program (ADAP) News

\What has changed?

Recently you may have received a letter from the State of Arizona (Arizona Department of Health Services) about changes to the drug assistance program called ADAP.  The ADAP program is reducing the number of free medications. To see the copy of the letter that was mailed to ADAP Clients (See Attachment English & Spanish)

When will the change happen?

The revised ADAP Formulary will begin July 1, 2009 and continue until further notice. We will inform you of any further changes. To view the revised see attachment  ADAP Formulary.

Where do I go to learn more about the ADAP changes?

  • You may contact your case manager, your medical provider, or a service agency.
  • You may contact ADAP at (602) 364-3610 or toll free at 1-800-344-1540 or visit the ADHS web site at http://www.azdhs.gov/phs/hiv/adap.htm.
  • If you are a patient at the McDowell Healthcare Center you may call (602) 344-6571.
  • If you are patient at the McDowell Healthcare Center and enrolled in Medicare Part D, please call Eric Moore at (602) 344-2632.

If I am a patient at McDowell Healthcare Center what are my options?

Ryan White Part A Patients

Because you are a patient the McDowell Clinic, if you will need to buy some of these medications, MIHS has a plan to help you. MIHS enrolled you in its CopaCare Plan so you can buy medications at a lower rate. CopaCare is a plan that offers lower priced medications based on your income. This plan allows you to get some medications at MIHS pharmacies for less money.

A letter will be mailed to you that includes a list of your personal medications. Attached is the letter and attachments that went out to McDowell patients. (See Attachment English & Spanish )

Your letter will have an attachment with a list of ADAP medications you have received. This list will tell you what medications are free and what medications you may have to buy after June 30th.

You can buy the medications from any of our nine pharmacies (See Attachment English & Spanish).

Medicare Part D Patients

If you are currently enrolled in both ADAP and Medicare Part D this information may help you.  This change may or may not affect the cost of medications you currently get from ADAP.

A letter will be mailed to you that includes a list of your personal medications. Attached is the letter and attachments that went out to McDowell patients enrolled in Medicare Part D. This list will tell you what medications are free and what medications you may have to buy after June 30th. (See Attachment English & Spanish forth coming)

The Arizona Department of Health Services (ADHS) has sent letters to ADAP participants with information regarding drugs available through the Apothecary Shop. Some medications that were removed from the formulary list might still be available to you (at no cost) for a limited time.

If you are low income and not eligible for AHCCCS, Arizona's Medicaid program, you may be eligible for ADAP. The state ADAP program provides medications to HIV positive people who have no health insurance and no other way to pay for the medications they need.
